Conference Registration:
Registration fees are subject to a $75.00 late fee if not
received by April 4, 2012. Registration fees are non-refundable
after April 9, 2012.

Hotel Reservation:
Lodging reservations will be accepted by returning the hotel’s
reservation form via fax 760-931-7569 or e-mail to
reservations1@lacosta.com. The hotel cut-off date is March
12, 2012. Reservations received after the cut-off date will be
based upon availability of rooms.

April 19, 2012, 1:00 p.m. Shotgun - Legends Course. Green
Fees: $175.00 per person
